AzCHOW’S CHW Institute of Excellence is proud to present the return of our Fall Symposium for 2024!
This years theme is Reconnecting Arizona: CHWs as catalysts overcoming mental health isolation and loneliness.

Important Note: Participants must be present and actively participating. For the virtual training participants must have access to Zoom, have a camera and a functional microphone. During the training cameras should be turned on. Only the participants that register (fill in this form) and attend will receive their certificate of attendance and participation.
